Family Discipleship Survey Guide

Why Use This Survey?

Healthy church-to-home discipleship begins with understanding where families are today. This survey is designed to help your church gain a clearer picture of family discipleship by discovering:

  • How parents understand their God-given role in discipling their children.
  • What discipleship practices are already taking place in the home.
  • Where parents feel encouraged and equipped.
  • Where parents would appreciate additional support, training, and resources from the church.

The goal is not to grade parents, but to listen well so your church can more faithfully equip and encourage them as the primary disciple-makers of their children.

Using the Google Form

  1. Open this Google Form:

  2. Click the three-dot menu in the upper-right corner.
  3. Select Make a copy.
  4. Rename the form for your church.
  5. Customize the questions, instructions, or settings as desired.
  6. Click Publish.
  7. Click the link icon and copy the responder link.
  8. Share the survey link with the parents in your church through email, your church app, social media, or other communication channels.
  9. As responses are submitted, view the results by selecting the Responses tab.

Should the Survey Be Anonymous?

Whether to make the survey anonymous depends on your ministry goals.

Advantages

  • Parents may answer more honestly and candidly.
  • Responses are less likely to reflect what parents think the church wants to hear.
  • Results often provide a clearer picture of your church's overall strengths, needs, and opportunities for family discipleship.

Disadvantages

  • You will not know which parents completed the survey.
  • Ministry leaders cannot follow up personally with families who express particular needs or request support.

Choose the approach that best fits your church culture and the purpose of the survey. If your primary goal is to understand the overall health of family discipleship, anonymity may encourage more honest responses. If your goal includes providing individualized encouragement and follow-up, collecting names may be more beneficial. 

The default setting in the survey is to request a name and email address. If you want to make the survey anonymous, just click Settings in the navigation bar and delete that section of the survey.

Whatever the results of your survey, view them as an opportunity to be encouraged by God's work, to identify areas for growth, and to strengthen your church's partnership with parents in making disciples of the next generation.
